P PT. Bekaert Advanced Filtration (BAF) yang berada di Karawang memproduksi komponen sistem filter polimer, seperti spin pack, candle filter, leaf disc, gasket dan lain sebagainya. Permasalahan pada PT. BAF adalah masih tingginya presentase cacat produk spin pack sebesar 7,99%. Tujuan penelitian untuk mengidentifikasi serta menganalisis permasalahan dan kegagalan yang terjadi pada proses produksi spin pack, memberikan usulan perbaikan untuk meminimasi presentase cacat. Metodologi penelitian menerapkan metode FNUA (Failure Mode and Effect Analysis) dan MAFMA (Metode Multi Attribute Failure Mode Analysis). Tahap pertama dilakukan dengan metode FMEA berdasarkan tiga kriteria yaitu severity, occurrence dan detection untuk mengidentifikasi kegagalan-kegagalan yang memiliki nilai prioritas tertinggi berdasarkan nilai FRPN (Fuzzy Risk Priority Number) yaitu, 402 pada proses bending 90o dan beberapa penyebab terjadinya kesalahan setting mesin, kelalaian operator dan kurangnya pengawasan pada operator. Kernudian, penilitian juga dilanjutkan dengan penentuan penyebab kegagalan potensial dilakukan dengan pemberian bobot pada ke-empat atribut yaitu, severity, occurance, detection dan expecte cost. Pemberian bobot tersebut menggunakan AHP (Analytic Hierarchy Process) dengan logika fuzzy.Atribut pada Metode MAFMA dimasukkan sebagai level kriteria dalam struktur hirarki AHP, sedangkan penyebab-penyebab kegagalan menjadi level alternatif pada di kasus pada PT. BAF, menunjukkan bahwa bobot severity 0.25989, kriteria occurance sebesar 0.0594, kriteria detection sebesar 0.1468 dan kriteria expert cost sebesar 0.5350. Sedangkan penyebab kegagalan yang paling potensial adalah skill operator tidak merata yaitu, 0.3602, kesalahan setting mesin, yaitu 0.2297, operator lalai karena terburu-buru, yaitu 0.2654, dan kurangnya pengawasan operator, yaitu 0.1448. Dan basil kegagalan potensial dilakukan usulan perbaikan dengan membuat alat bantu Jigpass yang berfungsi mempermudah operator dalarn pengoperasian mesin press. P PT. Bekaert Advanced Filtration (BAF) located in Karawang manufactures polymer filter system components, such as spin packs, candle filters, leaf discs, gaskets and so on. Problems at PT. BAF is still high percentage defect of spin pack product equal to 7,99%. The objective of the research is to identify and analyze the problems and failures that occur in the spin pack production process, suggest improvement to minimize defect percentage. The research methodology applied FNUA (Failure Mode and Effect Analysis) and MAFMA (Multi Attribute Failure Mode Analysis) method. The first stage is done by FMEA method based on three criteria of severity, occurrence and detection to identify failures that have the highest priority value based on FRPN value (Fuzzy Risk Priority Number) that is, 402 in 90o bending process and some cause of machine setting error, operator and lack of supervision on the operator. Later, research was continued with determining the cause of potential failure by weighting on all four attributes, severity, occurance, detection and expecte cost. The weighting using AHP (Analytic Hierarchy Process) with fuzzy logic. The attribute in MAFMA method is included as criteria level in AHP hierarchy structure, while the causes of failure become alternative level in case of PT. BAF, shows that the weight of severity 0.25989, occurance criteria of 0.0594, detection criteria of 0.1468 and expert cost criteria of 0.5350. While the most potential cause of failure is uneven operator skill that is, 0.3602, engine setting error, that is 0.2297, operator negligent because of the rush, that is 0.2654, and lack of supervision operator, that is 0.1448. And the resultant failure bacilli made a repair proposal by making a Jigpass tool that serves to facilitate the operator in the operation of the press machine.
